Website with a simple form to submit that will store in database. Everything has been built, I just need 2 things. 1. Change some of the fields (e.g. name), there is about 5 fields in total, maybe 2 or 3 need to be changed 2. Finish setup. Site has been moved from another server, so just need database setup, and make sure everything is configured to send email.
