119717 Custom_Form

Form function for SPQ. Script must have the following requirements. 1-I need a form for users to fill out and submitted to only the recipients that matches the user's criteria by default below. 2-User's will also have a choice to select different City, State (optional Zip) User will first: Select the City Select the State Enter Zip (optional) Results should display to the user the only results as in example below that have matched the user's input. The results will have only retrieved the vendors contact first and lastname and email. So when the user completes the form and clicks submit, it will only go to those vendors from that location. So every form submitted depends on the user's selection. Example: There are 25 vendors in San Diego, CA, 91920 location that you selected. button [reset] button [Next] button [Cancel] If user clicks on Next then is taken to the form for completion .User form must also have to be able to upload jpg, gif, bmp, eps, ai files. (I will send the complete info on the form later) But these are the main things that I must have for this to work. With this in mind, I am going need a separate form for vendors to fill out and their data stored in a MySql database. I also will need somekind of Admin area for me to manage my Vendors information. Updates, the works to add, delete, send out emails etc. Bottom line is, whatever data that the user fills out on the form must only be sent to the location of vendors in that area ONLY. When the vendor receives the data from the user. His or her email should read: "Price Quote Request from SPQ". Body of email: Should display all the data from the user's form along with contact email for replying. When vendor replies back to user, I Must also have a copy for records. That's pretty much it.
