Contact Form and Booking Form for website

We need someone to create 2 contact forms for our real estate website, one form for Inquiries and second one for Booking. Booking form has most of the fields that Inquiry form has, but also allows to choose to pay booking fee via Paypal. Some fields in the forms would be required, others optional. The forms would include text fields, checklists, radio buttons etc. Required fields: Email address,Name, address, city, zipcode, country, phone, arrival date, departure date, number of nights, etc. We would like validation word peple need to enter to prove they are humans. Also, on arrival and departure dates it would be nice to have small calendar pop-up so people could just choose their date by clicking on it and it automatically fill into the form. We would like the form to be build in HTML and set up to send results to email address using PHP file. If you suggest something different, that's fine as long as we will be able to upload it to our server and it works. Each form needs to be on a separate page (separate file). After you bid, we will send you the link to the contact form of another website that we like, to see if you could do something like that for us.
