Hello, I'm making a website on wordpress. I'm seeking someone who can create a form which is bit tricky. The form will have details to be filled by user visiting my website-- like name, email address, importantly upload photo/video option & then checkout via paypal. A user will have an option to upload multiple photos/videos (max3); corresponding to the number of photos upload selected; the amount should change & ultimately should checkout with that amount via paypal. Also, importantly I want a database of all the forms submitted on my wordpress website. Such that I can see all the forms submitted by the users with their corresponding form field details. I have attached few files showing what exactly I'm looking for. I want to get this done at the earliest. Kindly, contact me asap. Thanks.
