Create a survey for a website (jewellery)

Hey designers, Can someone please design me a survey on a website I'm building now on a simple survey of jewellery? I need this survey to determine what my customer wants in terms of jewellery. My business is a jewellery subscription based business and every month we will send them a box of jewellery. But before they subscribe I want them to go through a set of questions so that I will be able to understand what they need.
