Webpanel for taking and storing notes

Hi, Looking for someone to code a panel that could be hosted on my webserver for storing notes of my accounts. Tabs would be organized as "Group" and "Account" and would work like dropdown menus/texboxes when clicked on them. Please see sketch for more details! The 17.04, 18.04 etc. behind the "Account" tabs are dates when I created the accounts. I added them just as an example. No need to add some calendar feature - I will simply type the dates in the "Account" title :) Also has to include: -Login-logout feature with multiple user access (+admin user that can add/delete users) -Import/export feature (to .csv) Bonus point for nice design (my sketch is very rough) - facebook-like design would be ideal :)
