SKILLS NEEDED - Website Design, AJAX, CSS, Wordpress, Joomla, Flash, Graphic Design, CMS, PHP 1.)DESIGN 1 - Create an exact replica of a Joomla/Wordpress webpages (2 webpages) to suit the needs of a healthcare website. ([login to view URL]) - The basic Design, Layout, Colors, Font, etc of the template would remain the same. However, the images and content will have to be modified to suit Medical/Healthcare Purposes. - Also a form will have to be incorporated into the design. THE CODE SHOULD NOT BE COPY-PASTED. - It should be re-written from scratch. - The code needs to be written in a simple, organized, easy to understand and Lucid style. However, the website should also retain the look, feel, and quality of the original. 2.) DESIGN 2 – CUSTOMIZATION AND REDESIGNING. - Existing webpages (3 webpages) from the website [login to view URL] need to be modified to make them cross browser compatible and compatible across various screen sizes. - Also a professional and attractive animation/Flash File/Graphic, etc needs to be created to be used as a header (for a healthcare website). - Logo needs to be refined. - The columns need to be made of same length (currently the lengths are uneven). ---------------------------------------------------- FORMS- A few PHP forms with AJAX to be designed. [One short query form with Captcha + one Detailed query form with Captcha (with a feature for attaching multiple files) + one very-short form with Captcha that will be embedded by affiliates on their own websites + Forgot PW/Username Form]. Every time a query is sent, the IP address should be sent along with. An API/Code snippet needs to be designed so that any AFFILIATE should be able to embed our very-short query form on his own website so as to direct queries from his website to our's AND the end-user visiting the affiliate's website should be able to submit the form without having to leave the website of the affiliate. The details that the user will need to fill-up will include his name, treatment required, email, confirm email (should match the first email) and captcha. The details that should be emailed to us should include the name/URL of the website from where the query was directed, the city/state/country from where the query originated (to be extracted from the IP), the name of the end-user, the treatment required by him and his Email ID. Same forms will have to be customized as per the color scheme, font and designs of both the websites. INSTANT VALIDATIONS AND CONTEXT HELP (AJAX). Instant Validation and Context Help while filling out forms. While filling forms user should be prompted about the Correctness or Not of the Email-ID, age, phone number, etc, etc. IP ADDRESS - Both the websites will be posted on multiple domains. It should be possible to Allow and Dis-allow the websites being accessed from India.
