Provide a clean look code a PHP/MySQL website consisting of about 10 different pages/forms

I am building a website for small medical practices involving the following features: 1) Web front-end for PC/tablet based browsers (need CSS and clean look) 2) Web front-end for mobile devices (mostly operates exactly like the PC version, but with a couple pages that behave differently) 3) Login/Registration/Invitation of other users (most of this is already built, aside from the invitation of other users) - see this page for example of existing functionality: [login to view URL] 4) Minimal administrative categories for users (Owner, Administrator, Standard User) - this would result in more or less functionality/field access based on role 5) Several forms connected to a mySQL database: a) tblPractices (~ 3 fields) (some of this is already built) b) tblUsers (~ 6 fields) c) tblHospitals (~ 5 fields) d) tblPatients (~10 fields) e) tblCodes - allows user to search for and populate a field in tblPatients with results. The search functionality is already built, all that is needed is the saving when user exits the screen and returns to Patient view: [login to view URL] 6) Several lookup tables connecting the data: a) lkpUserToPractice b) lkpHospitalToPractice c) lkpPatientToHospital d) lkpPatientToPractice 7) Several sortable/filterable list pages a) Show all Practices associated with a user and user can select one to proceed b) Show all associated Users to a Practice and provide ability to add/edit/delete (if they are Owners or Admins) (form listed above in #5b) c) Show all associated Hospitals to a Practice and provide ability to add/edit/delete (if they are Owners or Admins) (form listed above in #5c) d) Show all associated Patients to a Practice and Hospital and provide ability to add/edit/mark inactive/active (all Roles can do this)(form listed above in #5d) 8) One printable/PDF output view showing a variation of #7d 9) Change tracking for various updates to various fields 10) One month of version control for patient data 11) Secure code to prevent SQL injection attacks, etc. 12) Foundation for monetization - build in the ability to accept payment for subscription services that can be turned on at a later date If your estimates come in the range I am expecting, I will provide mockups of all pages along with field mappings and functionality rules and logic.
