Using PHP to create dynamic PDF from SQL data source

We have a phone call charge tariff in a MS SQL database. We need a php script which will build a table in a pdf document based on this tariff. This way, it's easy for us to update the document when our tariff changes. We'll provide you with a Word doc and PDF file showing the output that we want. We'll also provide you with the connection info that you need to connect to the MS SQL database. We intend to host the php script on a Windows web server.
