MQTT design overview

One of my latest projects (completed) was to implement an MQTT client on arduino. The arduino solution was two fold: 1) A wifi enabled arduino (ESP8266). 2) A basic arduino with a GPRS module. Both solutions needed to be able to publish to a mqtt broker and subscribe to a topic of a MQTT broker , in order to be able to receive and transmit messages. Standard libraries with minor modifications were used. Both of the solutions required use of ssl enable mqtt brokers. I have NOT worked on setting the mqtt broker itself just the client side. I have not worked with Ethernet shield but due to the library architecture the solution should be similar (if not identical) to the wifi solution. The GSM solution is more complicated. Depending on your needs and the hardware you are going to use I might be able to help you.
