Report Generator

I am a school psychologist and would like to create a program that will allow me to enter in information/data about a client and select appropriate instruments used and recommendations (from a larger database) to be included in output - a final report. I guess the job would entail creating a "form" to fill out and check off certain selections that would then produce a printable report. This is also something I would be willing to create myself if their were some type of software that would guide me through the process of creating a form... Any suggestions or bids would be appreciated. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Complete ownership and distribution copyrights to all work purchased. ## Platform The program needs to run on Windows Millenium and newer, if possible
